CardioHealth is a secure, HIPAA-compliant software application designed to identify, educate, and track patients at risk for cardiovascular disease, diabetes, metabolic syndrome, hypertension, sleep apnea and risk factors related to coronary artery disease and sudden cardiac death.  The software tools are built around standard data interaction methods using an intuitive, low maintenance, Web 2.0 interface allowing for rapid learning and adoption by users with a wide range of technical abilities.  Medical professionals can easily enter the data they want to track, get to the results they need, and provide patients with the highest and most consistent level of care.  

On a clinical level, CardioHealth exists at the center of a web of related clinical specialties. Intimal Health’s software platform integrates data and insights offered by all of these specialties, as depicted in the diagram at right, to present a single unified view of cardiovascular best practices for the patient.

Designed to complement existing electronic medical record (EMR) solutions, CardioHealth presents clinicians with standard of care guidelines, reminders, and associated care options tailored to the specific patient’s needs, as revealed by their individual data, to more consistently and effectively manage the patient’s disease state and future risk factors.  It is also customizable, with guidelines and recommendations that are easily edited to meet the specific care methodologies of a particular practice or hospital.  As a result, CardioHealth drives consistent, best-in-class cardiovascular care for all patients and enables enhanced patient outcomes though longitudinal tracking of their progress.

CardioHealth integrates with existing EMRs as an adjunct application, which is a critical differentiator since many practices have already invested heavily in EMRs that do not offer comprehensive disease management programs.  It can be populated with patient data using several methods:  by integrating with existing databases and sources using standard data interfaces; integrating with existing databases and sources through custom-built solutions; or by manually entering patient data.  Ease of integration with existing patient data sources removes a significant barrier to user adoption and offers a significant competitive advantage over less robust software offerings.  Once populated, the CardioHealth data can be searched with an intuitive, easy-to-use query tool that can identify groups of patients in need of additional interventions, those who might fall under new guidelines, or as a clinical research tool.

From a standards compliance standpoint, CardioHealth incorporates the current standards outlined in The Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure (JNC VII), and evidence-based guidelines developed by the American College of Cardiology (ACC), to create an algorithmic comparison between these accepted standards of care and the patient’s history, symptoms, and medical test results.  Additionally, CardioHealth meets all the required components to be considered a comprehensive diseases management program as outlined by the Disease Management Association of America (DMAA) in the American Healthways & Johns Hopkins Standard Outcome Metrics and Evaluation Methodology for Disease Management Program report.
