MyCardioHealth is a portable USB device designed to assist and engage patients and family members in learning about coronary disease and the steps they can take to reduce risk factors and improve their cardiovascular health (a patient education web portal is also under development).  MyCardioHealth provides patients with customized diet, education exercise, and risk assessment modules which help enhance treatment compliance by providing patients with the required resources and motivation to attain their goals.  It also provides patients with an easy method to store a wide range of medical records, share these records with other health care providers, and longitudinally track their progress toward specific goals (see Appendix 1 for example patient reports).  Finally, when the development of the patient portal is complete, Intimal Health will be able to engage, upon the patient’s approval, their primary care provider by granting them access to the patient’s information in order to facilitate better coordination and avoid the numerous “hand-off” problems that often develop between specialists and primary care.

Logistically, when the patient’s appointment with their cardiologist is complete, the patient’s specific information - including their complete care plan, recommended diet and exercise program, lifestyle modifications and other educational material tailored to their personal health history, known risk factors, and medical data – is printed and reviewed with the patient and is also downloaded from the CardioHealth platform and stored on the patient’s MyCardioHealth USB flash drive.  This information is then accessible from any personal computer with a USB port.  Patient engagement in their treatment plan has been shown to have a significant and positive role in attaining desired cardiovascular outcomes.    

The patient portal, when complete, will be co-branded with, and marketed as a value-added offering to, CardioHealth and MyCardioHealth customers.  Pricing options for the patient portal are being considered, but could include options such as initial user access fees, subscription fees, or advertising revenue.
